Instructions:

You need to print out and complete two forms for proper registration with the Special Olympics, and you can download them from this page. We have combined the forms into a single file, so you need only download and print one file.

The download package contains the Application for Participation in Special Olympics , and it contains 2 Official Special Olympics Release Forms. One release form is for an athlete that is signing for themselves, the other is for an adult partent/guardian to sign on behalf of an athlete. Only one of the release forms needs to be returned with the Application for Participation. We also included instructions for completing and mailing the forms.

These files are in Adobe Acrobat (.pdf) format; in order to print the file you need Adobe Acrobat Reader installed on your computer . This program is free, and is available from the Adobe web site. Acrobat is the only program that works consistently on all platforms (Macintosh, Unix, Windows), and that will insure that the forms print correctly no matter what type of printer you are using.

If you spend any time at all on the Web, you will need this program sooner or later. Instructions for downloading and installation are available at the Adobe site.

So you need to:

1. Download and install Adobe Acrobat Reader.
2. Download the Registration Forms file, below.
3. Open that file (sovaregform.pdf) using Acrobat Reader.
4. Print that file on your printer.
